Our designated registered nurse or therapist will visit your home for an initial assessment to determine your needs and health goals. They must also make sure you understand the services, their benefits, and the availability of resources you can find in your community. The discussion and gathered information will be relayed to your physician to coordinate effectively.
The assessment is the primary phase wherein information about the patient and other factors are recorded to be used for the planning of the individualized care plan. The documentation will include:
- Current Health Conditions
- Medical Diagnoses
- Functional Abilities/Disabilities
- Head-To-Toe Assessment
- Medications
- Overall Nutrition and Appetite
- Ability to Carry Out Daily Living Activities
Other factors include:
- Mental and Emotional Health
- Home Environment
- Social Engagement
- Safety Issues
- Support Network
For better information gathering, we encourage our patient’s family members to participate and be involved. Afterward, we will coordinate with the patient’s primary care physician (PCP) to determine the schedule and frequency of visits we must conduct and the services we must provide to help them regain their strength and independence at home.
Home Health Care Monitoring
In addition to the services we provide, such as our home health care therapy, nursing, and other services, we also ensure the treatment progress effectively through regular monitoring. We must also monitor if they are slowly regaining or maintaining their independence. The results of the monitoring will determine whether the patient’s care will change, continue, or end.
